Panel on Treatment Resistant Conditions

CE Hours 2

About this live event

This panel will discuss novel interventions for treatment resistant conditions. Dr. Ziv Cohen will review how to identify patients with treatment-resistant depression who may benefit from interventional psychiatric treatments, including transcranial magnetic stimulation (TMS), ketamine, and electroconvulsive therapy (ECT). Participants will learn practical criteria for recognizing treatment resistance, assessing clinical severity, and determining when referral for interventional evaluation is appropriate. The session will also discuss the indications, patient selection considerations, and comparative roles of TMS, ketamine, and ECT within evidence-based treatment algorithms for depression. Dr. James Murrough will review the clinical burden of major depressive disorder and treatment-resistant depression, highlighting the limitations of conventional monoaminergic antidepressants that spurred the search for rapid-acting alternatives. The talk will trace the discovery and mechanism of ketamine and esketamine, including pharmacology and clinical efficacy and safety, in treatment- resistant depression and depression with suicidal ideation. The talk then turns to classic psychedelics, such as psilocybin, examining their proposed 5-HT2A receptor-mediated mechanisms of neuroplasticity and how the emerging mechanistic and clinical evidence compares and contrasts to that for ketamine. Drawing on the Montreal Model of ketamine-assisted psychotherapy, whose motto is "feel your feelings, defuse your thoughts, change your behaviors," Dr. Joseph Diehl will describe how clinicians can use familiar CBT tools before, between, and after dosing sessions, and which patients may be the best fit. He will also discuss conditions beyond depression, including OCD, anxiety, and substance use, and what the current evidence does and does not yet support.

Learning Objectives

  • Summarize the clinical efficacy and safety data for ketamine and esketamine in treatment-resistant depression and major depressive disorder with suicidal ideation, including mechanistic insights from glutamatergic and neuroimaging studies.
  • Evaluate the emerging evidence base for classic psychedelics (e.g., psilocybin) in depression, including proposed 5-HT2A receptor-mediated mechanisms of neuroplasticity and how these compare to ketamine's mechanism of action.
  • Identify practical criteria for recognizing treatment resistance, assessing clinical severity, and determining when referral for interventional evaluation is appropriate.
  • Learn how to prepare a patient for ketamine or psychedelic treatment using familiar second- and third-wave CBT tools, including a functional analysis of the patterns they want to change, concrete behavioral goals, and practice approaching difficult sensations and thoughts rather than avoiding them.
  • Determine how to treat the weeks after dosing as a window of opportunity, using exposure, behavioral activation, and relapse-prevention strategies to help new perspectives become lasting change in depression, OCD, anxiety, and substance use.

Live event Instructor(s)

  • Ziv E. Cohen, M.D., F.A.P.A.

Ziv J. Cohen, MD, is the Founder and Medical Director of Principium Psychiatry, a multidisciplinary psychiatric company specializing in mood and anxiety disorders, interventional psychiatry, and forensic psychiatry. A board-certified psychiatrist and forensic psychiatrist, Dr. Cohen has particular expertise in the evaluation and treatment of complex mood and anxiety disorders, including treatment-resistant depression.

Dr. Cohen founded Principium Psychiatry to provide sophisticated, evidence-based psychiatric care that integrates psychopharmacology, psychotherapy, measurement-informed treatment, and advanced interventions including TMS, ketamine, Spravato, and ECT. Principium also provides forensic psychiatric evaluations, bringing rigorous diagnostic assessment and psychiatric expertise to legal and occupational questions.

In addition to his clinical and forensic work, Dr. Cohen is involved in physician education, quality improvement, and the development of clinical systems that improve outcomes for patients with complex psychiatric illness. His teaching focuses on practical, evidence-based decision-making in mood disorders, anxiety disorders, treatment-resistant depression, interventional psychiatry, and forensic psychiatry.

  • James W. Murrough, M.D., Ph.D.

Dr. James Murrough is Professor of Psychiatry and Neuroscience, Vice Chair for Clinical Research in the Department of Psychiatry, and Director of the Dennis S. Charney, MD, Depression and Anxiety Discovery Center at the Icahn School of Medicine at Mount Sinai in New York. He has made major contributions to biological psychiatry and clinical neuroscience, particularly through his work advancing ketamine and other novel therapeutics for treatment-resistant depression (TRD) and posttraumatic stress disorder (PTSD), and is internationally recognized for his research in depression, anxiety, and PTSD. He is a Fellow of the American College of Neuropsychopharmacology (ACNP), and his honors include the Joel Elkes Research Award from ACNP, the A.E. Bennett Award from the Society of Biological Psychiatry, and the Paragon Award for Research Excellence from the Doris Duke Foundation. He sits on the Scientific Council of the Brain & Behavior Research Foundation and serves as Vice Chair of the Scientific Council of the Anxiety and Depression Association of America. Alongside his research, Dr. Murrough maintains an active clinical practice and is an avid mentor to trainees and junior faculty.

  • Joseph Diehl, PhD

Joseph Diehl, PhD, is a postdoctoral fellow in the Psychedelics and Contemplation Lab at McGill University, where he works with Drs. Kyle Greenway and Nicolas Garel, developers of the Montreal Model of ketamine-assisted psychotherapy for treatment-resistant depression. He received his PhD in clinical psychology from Duke University and completed his predoctoral internship at Weill Cornell Medicine/NewYork-Presbyterian. His clinical training includes DBT, behavioral activation, and mindfulness-based cognitive therapy, and his research examines the processes, such as mindfulness, perseverative thinking, and the therapeutic alliance, that help explain who benefits from psychedelic and ketamine treatments and why. His recent articles include a process-based therapy framework for psychedelic-assisted psychotherapy and of a study of the therapeutic alliance in MDMA-assisted therapy for social anxiety disorder, and he created the Montreal Model's first online clinician training module.
